Small Event Grant Application

A final report of the event is to be submitted to the SDA Awards Administrator within one month following the conclusion of the event, including:

Assessment of the event’s success:
- Number of attendees/participants
- New SDA members gained
- Detailed financial account of profit/loss
Copies of invoices, receipts, canceled checks, etc., for the expenses covered by the grant
Any learnings gleaned from the experience
Copies of event-related materials

Submit a completed W-9/W-8 form prior to receipt of grant funds. If this requirement presents an issue, please contact us at awardsadministrator@surfacedesign.org

If your proposed event is funded but then later canceled, the full grant amount must be returned to SDA.
